I am a dependent of my husband's on an insurance company that is affiliated with his company.

The insurance plan we have is called "High Deductible," which means that we have a set amount of annual out-of-pocket that the insurance company will pay if we exceed that amount. (Annual limit: $4,000)

Since it was the beginning of the year, I had to pay the out-of-pocket limit ($4,000). I also had a Health Saving Account ($1,000) that I had accumulated so far, so I allocated that to the payment and paid the rest ($3,000) in monthly installments.
